Must live in Seattle city limits, Skyway, North Tukwila (north of S 120th St), or North Burien (north of SW 148th St).

Description
Determines eligibility for early intervention services. This includes an Individual Family Service Plan of goals and a full developmental assessment/screen in five areas of development as well as any developmental area of concern.
