Description

Community-based cardiovascular health promotion program that provides community education and multi-risk factor screening (including blood pressure, cholesterol and glucose measurements).   Offers follow-up and referral services to residents of King County.   Also provides community education on cardiovascular health-related topics including blood pressure, cholesterol and nutrition.

Provides assistance to immigrants and refugees in accessing basic and emergency resources, naturalization, ESL instruction, public benefits and health insurance. Works to eliminate diabetes-related health disparities among minority communities by offering support groups, education and advocacy. Offers free HIV screening to eligible people of color.
